Penta’s Community-Based Transition of Care Model™will be featured in the next HHQI Best Practice Intervention Package to be released on October 28th, 2010. This transitions model was developed by homecare providers, for homecare providers and address gaps in all care transitions. Our article regarding homecare’s role in the Patient-Centered Medical Home Model was published in the Cleveland Clinic Journal of Medicine, titled: Bringing home the ‘medical home’ for older adults.
